Major & Minor Bone Grafting

Missing teeth over a period of time can cause your jaw bone to atrophy, or resorb. This often results in poor quality and quantity of bone suitable for the placement of dental implants as well as long term shifting of remaining teeth and changes to facial structure. Most patients, in these situations, are not candidates for dental implants.

Fortunately, today we have the ability to grow bone where it is needed. This not only gives us the opportunity to place implants of proper length and width, but it also gives us a chance to restore functionality and aesthetic appearance.

Major Bone Grafting

Bone grafting can repair implant sites with inadequate bone structure due to previous extractions, gum disease, or injuries. The bone is either obtained from a tissue bank or your own bone is taken from the jaw, hip or tibia (below the knee). Sinus bone grafts are also performed to replace bone in the posterior upper jaw. In addition, special membranes may be utilized that dissolve under the gum to protect the bone graft, as well as encourage bone regeneration. This is called guided bone regeneration, or guided tissue regeneration.

Major bone grafts are typically performed to repair defects of the jaws. These defects may arise as a result of traumatic injuries, tumor surgery, or congenital defects. Large defects are repaired using the patient’s own bone. This bone is harvested from a number of different areas depending on the size needed. The skull (cranium), hip (iliac crest), and lateral knee (tibia), are common donor sites. These procedures are routinely performed in an operating room and require a hospital stay.

Dr. Jessica Lee comes with my highest recommendation. Both of my daughters required oral surgery to attach brackets to teeth up in the gums to help bring them down / expose them. The consultation itself included 3-D X-rays and a decent amount of Dr. Lee's time. This was immediately helpful, insightful and pleasant; we left the office with answers to all of our questions that had been lingering for months prior, as well as relief & confidence that the procedures would go smoothly. And they did. The surgeries themselves were successful, quick, according to plan - all handled with skill and precision. On a return overseas flight a year ago, completely out of the blue, I experienced excruciating ear pain as if someone was stabbing ice picks in both of my ears. I have NEVER had a problem flying, and I was beside myself in pain. When I got home, I immediately went to my PCP to try to find out what was going on. One of my eardrums was a little swollen, but it didn’t warrant medication. Over the next three months, I went back to my PCP’s office two more times, seeing two different PA’s who still could not determine a cause: at this point I couldn’t eat anything but a soft diet, I couldn’t talk without pain, and it was impossible to sleep because each side of my head was so painful, even to touch. On the third visit, the PA gave me some ear drops and said that if they did not make a difference, I would need to see an ENT. When there was no change, it had been three months since the instantaneous onset of symptoms. So I left the office to schedule an appointment with an ENT. It took less than 8 seconds for the ENT to declare, “You don’t have an ear problem; you have TMJ! You’ll need to see an oral surgeon.” I’ve NEVER had a TMJ issue of any kind that I knew about. I sat in the ENT’s parking lot, called my dentist for a referral for oral surgeons, and got on the phone immediately with all 6 of them. I could only get an appointment 2 weeks out with one of them, and then called Dr. Jacob’s office. He asked me, “Can you be here in 10 minutes?!” I flew to his office and received incredible care right away. The photographs and panoramic X-rays revealed that both condyles in my jaw were disintegrating. His diagnosis was that I would require a bilateral TMJ joint replacement surgery to correct the issue. The prosthesis would be created at a laboratory in Ventura, CA, and the surgery was scheduled for several months out. All of this was pre-Covid which ultimately altered some of the original plans. In a subsequent visit, more pictures were taken and compared to the originals indicating further deterioration. Measurements were taken and a mold created to make the prosthesis: a 3-month long process. I knew no one who had ever had this kind of surgery, and it was a pretty terrifying prospect. I began working with a rheumatologist as well, in hopes of discovering the cause of my bone disintegration and whether it was caused by a systemic arthritis of some sort. The jury is still out on that, even post-surgery and the pathology report. I chose Morristown Medical Center for my procedure. The surgery was long: 5 ½ hours, and it required one night in the ICU and a subsequent night in a step-down unit. But the care I received was exceptional, and my surgical team from Northeast Facial and Oral Surgery Specialists, LLC: Dr. Jacob, Dr. Aziz and Dr. Lee did astounding work and very attentive post-surgery care! Fortunately, I had three physical therapy sessions before my surgery which made a HUGE difference in my recovery. About 2 days post surgery, I was able to begin PT, using the exercises I had already learned. I resumed PT with my physical therapists 3 weeks post-surgery, and am working hard to improve and restore muscle control and speech issues. Trying to find an oral surgeon to do this particular surgery was difficult; I did go for a second opinion, and I could not find a surgeon in North Jersey who would even attempt to do it, it is so intricate and specialized. I am SO very thankful I discovered Dr. Jacob and his team. I cannot recommend their expertise enough! The recovery is not easy, and there have been significant challenges over the past couple of weeks with eating and typical recovery issues from an extensive surgical experience. But I trust that I will continue to receive exceptional care and guidance going forward, and would absolutely recommend Northeast Facial and Oral Surgery Specialists, LLC to anyone who requires this particular TMJ joint replacement or other oral surgery procedure.
